Neelam should not be worn straightaway. First take neelam and keep it under your pillow and if you see horrible dreams, or fever, or any problem during night, Nilam is not fit for you.

if it did not create any problem, then you can wear it after bathing it for 108 times with milk while reciting Om Shaum Shanishcharaye nama
